Spent yesterday at a community health fair in a low-income neighborhood. Five hours, ~80 patients, mostly screening for hypertension and diabetes. Half had not seen a clinician in over a year. The follow-up logistics are the hardest part — we can identify the problem, but the bridge to ongoing care is fragile. Working on a better referral pipeline for next month's event.
